Option 2: You can also make an appointment by filling out information below. In the “Reason for visit” box, list your chief complaint and also your physical address (important for filling prescription with pharmacy).

We will need your driver’s license to verify you. If you insurance with HMSA, UHA, or HMAA/HMWG please have your membership card available as well.

Our staff will reach out for additional information if needed and we will also send you an invitation to OnPatient.com as a patient portal.
